If you have been told that you have cataracts in both eyes and have been advised to consider cataract surgery, it is completely natural to have questions and concerns. It is important to understand what cataract really is, when surgery is actually required, what the different lens options are, and what you can expect after surgery.
I have written this comprehensive article to walk you through every aspect of cataract and help you make a well-informed, comfortable decision about your eyes.

What Is a Cataract?
A cataract is simply a clouding of the natural lens inside the eye. The natural lens is normally crystal clear and allows light to pass unobstructed through the pupil to form a sharp image on the retina at the back of the eye.
When the proteins within the lens undergo changes and become cloudy, light cannot focus properly, and your vision gradually becomes blurred or hazy. Cataracts can affect both distance vision (looking at road signs or TV) and near vision (reading).
Clinical Insight: The "Second Sight" Phenomenon
Interestingly, certain types of cataracts can sometimes make near vision appear better for a period of time. In my OPD, I often meet patients in their 60s who tell me that they can now read the newspaper without their reading glasses! However, this does not mean that the cataract is getting better.
As the cataract becomes harder, browner, or denser, near vision can temporarily improve due to optical shifts in the refractive power of the lens. This temporary change should never be mistaken as a reason to avoid or delay surgery if the cataract is otherwise affecting your eye health.
When Does a Cataract Need to Be Operated?
A very important point I always emphasize to my patients: Not every cataract requires immediate surgery.
Cataract surgery is generally considered when:
- Daily Interference: The cataract is actively interfering with your vision, driving, computer work, or daily activities.
- Lens Hardness: The cataract has become very hard or dense.
- Retinal Health: The cataract needs to be removed to allow clear visualization and laser/surgical treatment for an underlying retinal condition (such as diabetic retinopathy or vein occlusions).
- Reduced Dependence on Glasses: You wish to reduce your dependence on spectacles and are otherwise a suitable candidate for refractive cataract surgery.
Therefore, the decision to undergo cataract surgery should not be based simply on the presence of a cataract. It should be based on the condition of the cataract, the quality of your vision, your symptoms, and the overall health of your cornea and retina.
What Are the Symptoms of Cataract?
Cataract can produce a variety of symptoms, and these can differ from person to person depending on the type and location of the lens clouding:
1. Blurred Vision
The most common symptom is blurred or hazy vision. In an early cataract, you may not notice any significant change in your daily vision. You may only discover that your vision has reduced by one or two lines during a routine eye examination. If cataracts are present in both eyes to a similar degree, many people do not notice the gradual change because the deterioration occurs slowly. However, some patients can experience a dramatic reduction in vision, particularly when the central posterior part of the lens becomes cloudy.
2. Glare and Difficulty Driving at Night
Cataracts can cause significant light scatter, leading to glare, starbursts, and halos—particularly in bright sunlight or when looking directly at oncoming vehicle headlights at night. Night driving often becomes increasingly difficult because of glare and reduced contrast.
3. Difficulty Seeing Clearly (Foggy Vision)
As the cataract progresses, vision can gradually deteriorate. Patients describe their vision as foggy, hazy, or generally poor, as if looking through a steamed-up window.
4. Double Vision in One Eye (Monocular Diplopia)
A cataract can occasionally cause monocular double vision, meaning that double or multiple ghost images are perceived in one eye even when the other eye is covered.
Can Young People Develop Cataracts?
Yes. Cataract is not exclusively a disease of old age. Younger individuals can also develop cataracts and may require cataract surgery.
There can be several reasons for cataracts developing at a younger age, including diabetes, previous eye injuries, steroid use, uveitis, or previous retinal surgery. The eye should be carefully and thoroughly examined to determine the cause and to assess whether there are any associated eye conditions.
Can Eye Drops Cure Cataract? (A Direct Warning)
Beware of Misleading Claims
At present, there is no scientifically proven eye drop that can reverse, dissolve, or cure an established cataract.
You may come across advertisements or news reports claiming that cataracts can be dissolved using eye drops. These claims should be approached with extreme caution. Once the natural lens has become significantly cloudy, the only safe, permanent, and definitive treatment is surgical removal of the cloudy lens and replacement with an artificial intraocular lens (IOL).
What Is the Treatment for Cataract?
The definitive treatment for a visually significant cataract is cataract surgery.
One of the most commonly performed and modern techniques is called Phacoemulsification. During phacoemulsification:
- A very small micro-opening (typically around 2.2 mm) is made in the edge of the eye.
- A specialized ultrasound instrument is used to gently break up and remove the cloudy natural lens.
- An artificial foldable intraocular lens, or IOL, is then implanted in its place.
Is Cataract Surgery a Major Operation?
Modern cataract surgery is a minimally invasive and generally very safe procedure. It is usually:
- Sutureless (no stitches required)
- Performed through a very small micro-incision
- Completed within approximately 5–10 minutes in uncomplicated cases
- Performed under topical anaesthesia, meaning numbing eye drops are used and an injection around the eye is not required
When the cloudy lens is removed successfully and an appropriately calculated intraocular lens is placed correctly, visual recovery can be excellent. Many of my patients experience a significant improvement in their clarity within the first 24 hours. Some patients may experience mild temporary corneal swelling or inflammation, which usually settles over several days.
Is Cataract Surgery Safe? & Recognizing Eye Emergencies
Cataract surgery is one of the most commonly performed surgical procedures in the world and has a very high safety profile. However, like any surgical procedure, it is not completely free of risk.
One of the most serious but rare complications is an infection inside the eye called endophthalmitis. Possible risk factors include contamination during surgery or recovery, or underlying conditions such as uncontrolled diabetes. Although the risk is very low, patients should be aware that any sudden deterioration or unexpected change in vision requires urgent evaluation.
Sudden Vision Loss: Why Prompt Action Is Vital
Sudden vision loss in one eye can be a sign of a serious condition affecting the retina or the optic nerve. Do not wait for vision to improve on its own. Prompt diagnosis and treatment are essential to prevent permanent loss of vision.

Which Intraocular Lens (IOL) Is Best for You?
Choosing the correct intraocular lens is an essential and personalized part of cataract surgery.
The first step is accurately calculating the power of the intraocular lens to be implanted. This requires precise biometry measurements using advanced technology. If the incorrect lens power is implanted, you may not achieve the intended refractive correction and may need glasses after surgery.
However, the type of lens you receive is equally important. In my practice, the choice depends on several clinical factors:
- The condition of your retina and macula
- The health of the posterior segment of your eye
- Whether you have glaucoma or increased eye pressure
- The health and curvature of your cornea
- The overall health of your eye
- Your specific visual requirements and lifestyle expectations
Monofocal or Unifocal Lens
A monofocal lens is designed primarily to provide clear vision at one particular distance—in most cases, selected for good distance vision. You will then require glasses for reading or close work.
There are different designs and optical characteristics available. A monofocal lens is often the safest and best choice for patients with underlying retinal conditions, diabetes, or glaucoma.
Multifocal & Premium Lenses
Multifocal lenses are designed to provide vision at more than one distance, reducing dependence on glasses for both distance and near vision.
However, multifocal lenses are not suitable for everyone. The retina, macula, optic nerve, and cornea must be completely healthy. If there is underlying retinal disease or glaucoma, multifocals may cause glare, halos, or reduced contrast sensitivity.
Why I Always Check the Retina Before Recommending Premium Lenses
As a retina specialist, I always perform high-resolution OCT macula scans prior to cataract surgery. If there is early diabetic macular edema or macular degeneration, putting a multifocal lens can compromise your contrast vision. In such cases, a high-quality monofocal lens gives far superior, clearer visual comfort.
What Happens After Cataract Surgery? Postoperative Precautions
Postoperative care following uncomplicated cataract surgery is generally simple. Specific instructions and eye drops will be prescribed:
Important Postoperative Precautions:
- Avoid water entering the operated eye for approximately one week, or for the period recommended.
- Avoid head baths during the initial postoperative recovery period.
- Clean your face gently using a clean, damp wipe without splashing water.
- Use all prescribed eye drops exactly as directed.
- Keep your eye clean and do not rub or apply pressure.
- Keep your blood sugar well controlled if you have diabetes.
- Avoid eye makeup, kajal, and similar products for approximately one month.
- Avoid swimming until confirmed safe.
- Avoid activities or contact sports where there is a risk of contamination, injury, or trauma.
Most importantly, pay attention to your vision after surgery. If your vision suddenly becomes blurred or deteriorates after initially improving, or if you develop pain, redness, discharge, flashes, or floaters, contact us immediately.
